Total & Permanent Disability Discharge
Your loans may be forgiven if you become totally and permanently disabled and are unable to earn money because of an injury or illness that is expected to continue indefinitely or to result in death. .
Eligibility:
- For borrowers who are unable to work & earn money due to an illness or injury that is expected to continue indefinitely or result in death.
- Condition may not have existed at the time the loan(s) was applied for, unless the condition later worsened to the point of total & permanent disability.
- To be eligible, there can be no disbursements after the date the doctor certifies the Borrower is no longer able to work.
- Form must be an original. Faxes are not accepted.
- Form must be signed by borrower or borrower's representative.
- Physician's section must be completed and signed by a doctor of medicine (MD) or Osteopathy (DO) who is licensed to practice in a state. [The definition of "state" from the discharge form: State includes the 50 United States, the District of Columbia, American Samoa, the Commonwealth of Puerto Rico, Guam, the Virgin Islands, the Commonwealth of the Northern Mariana Islands, the Republic of the Marshall Islands, the Federated States of Micronesia, and the Republic of Palau.
- Applications must be returned within 90 days of the physician's signature.
